Home Support Assessor and Support Officer

An opportunity is available for an experienced, enthusiastic Home Support Assessment and Support Officer who has experience in home & community care for young people.

 

The Aged Services Unit undertakes strategic planning, policy development, advocacy and partnership activities.  Key service responsibilities include the provision of high quality assessment and home support services for older people, people with a disability and their carers and people eligible under the HACC Program for Younger People

 

About the role

You will conduct client assessment and review – for the Home and Community Care (HACC) Program for Younger People (PYP)

  • Establish client eligibility for receiving services through the HACC Program for Younger People (PYP).  Identify the needs and expectations of the client and their carer, setting up the client’s best pathway through the service system.

Administration

  • Ensure the strict security of all confidential client information, taking into account the Privacy Act and Health Records Act
  • Document accurate, comprehensive and current data on the Xpedite system 
  • Prepare reports as required.

Policy and Planning

  • Assist in policy and procedure development  and reviews as required

 

You will have

  • Tertiary qualifications in Nursing, Social Work, Allied Health, or related field
  • Experience in undertaking service specific needs identification, holistic client assessments, writing support plans and referring, particularly in the HACC PYP
  • Ability to collect information in a sensitive manner
  • Ability to manage and prioritise a variety of complex tasks  
  • Well-developed communication skills
  • Proficient IT skills – particularly with aged and disability services software
  • Current Victorian Drivers Licence
  • Working with children check
  • Evidence of COVID vaccination

The successful applicant must be prepared to undergo a police check.

Benefits

  • Salary range $86,182.74 to $93,696.04 pro rata plus super
  • Temporary part time 24 hrs per week November 2021 to June 2022
