ABOUT THE ROLE

Moreland City Council is offering a fantastic opportunity for an experienced Assessment Officer to join the Moreland Regional Assessment Team.   You will be responsible for undertaking assessments for the Home and Community Care Program for Younger Persons using the Statewide Living at Home Assessment tool. You will also be required to complete Home Support Assessments using the My Aged Care Assessor Portal.   Experience working in Aged and Disability sector would be an advantage, as well as experience or knowledge of the National Disability Insurance Scheme (NDIS).

SKILLS & EXPERIENCE REQUIRED TO PERFORM THIS ROLE

  • Qualifications in allied health or a relevant degree and experience in working with client with a disability or working with aged clients
  • Experience, knowledge and understanding of the Home and Community Care Program for Younger People, the use of reablement and wellness approach when working with clients
  • Highly developed assessment and care coordination skills using the Living at Home Assessment Tool or other similar tools
  • Experience in My Aged Care and Home Support Assessments 
  • Demonstrated experience in the development of Goal Directed Care plans
  • High level of written and verbal communications skills
  • Experience and knowledge of client data base system, Carelink+ is desirable
  • Ability to speak a second language is highly desirable
