ABOUT THE ROLE

The Aged and Community Support Branch is seeking a Home Support Assessor to work within the Assessment Services Unit. The position is responsible for undertaking Home Support Assessments utilising the NSAF and My Aged Care Portal.

We require a person with a qualification in Social Work, Nursing or other Allied Health, or Community Services degree. The successful applicant will have proven experience in My Aged Care, holistic person centred assessments and goal directed care planning.

The person we are looking for will have excellent communication skills, interpersonal and problem solving skills with the ability to manage own workload with good time management skills.

SKILLS & EXPERIENCE REQUIRED TO PERFORM THIS ROLE 

  • Experience working within the RAS is desirable.
  • Current driver’s licence and own car is required.
