The City of Stonnington has a fantastic opportunity for a professional Service Coordination Administration Officer to join our Care Services Unit.
As the Service Coordination Administration Officer you will be responsible for providing support and services to clients accessing the Commonwealth Home Support Program (CHSP), ensuring the transfer of data between databases is accurate and generating monthly reports to meet service delivery standards. Additionally this position will work closely with the Service Coordinator Officer to develop and deliver service plans which support client wellbeing goals including the monitoring of client progress as well as completing daily administration and management of the Council’s My Aged Care service provider portal.
To be successful in this position, you will have a qualification in aged care or community services, an understanding of issues related to older people, people with disabilities and their carers and excellent verbal and written communication skills.
