Are you an enthusiastic health or community service professional who is passionate about assisting older people and people with a disability to access supports?

Darebin City Council's Aged and Disability Department is a Regional Assessment Service (RAS) outlet for My Aged Care and the HACC Program for Younger People. We are seeking an enthusiastic qualified professional to join our dynamic assessment team.

The successful candidate will have high level customer service and organisation skills required to perform skilled triage and referral management for both assessment programs. They will also participate in community-based client assessment, support planning and reviews; requiring competence in strengths-based, person-centred and diversity-aware assessment frameworks.

The incumbent will be proactive in identifying inefficiencies and collaboratively developing solutions to improve systems, work processes and practices. Situated in a highly skilled assessment team, the incumbent will be supported to develop and refine skills in client assessment, support planning and review.
