Whitehorse City Council currently has a vacancy for an experienced Home Support Assessor to conduct holistic client assessments, prepare individually-tailored support plans and make appropriate service referrals to assist people to maintain active and independent lives in the community. Knowledge of relevant aged care and disability programmes including the Commonwealth Home Support Programme (CHSP) and Victorian Home and Community Care Programme for Younger People (PYP) is highly desirable.
