Access and Support Officer

• Band 5 – $68,544 - $78,480 pro rata + Super
• Temporary 12 month tenure – Maternity Leave Replacement
• Part time 4 days per week (0.8EFT)

An exciting opportunity is open within the Whitehorse City Council Home and Community Services Department for a Consumer Access and Support Officer.

Whitehorse City Council supports consumers over 65 years, eligible for Commonwealth Home Support Programme

This position will be responsible for the establishment, coordination and review of services with consumers accessing services from Whitehorse City Council. 

As a Consumer Access and Support Officer at Whitehorse City Council you will:
• Develop and review consumer care plans, consistent with an emphasis on capacity building, physical and mental wellness and social inclusion
• Facilitate timely service implementation and referrals
• Apply relevant program fees, policies and service agreements with consumers in consideration of their individual circumstances
• Work collaboratively with consumers, their advocates and relevant stakeholders, encouraging feedback, to enable best practice and service provision in relation to the consumers’ individual needs
• Respond in a timely manner to arising issues, requests and/or concerns in relation to consumers circumstances or services in place

Whitehorse City Council Home and Community Services Department has a long standing and strong commitment to supporting consumers to remain living independently at home. The Consumer Access and Support Officer will work towards maintaining and further strengthening the working relationships with consumers and their carers. This role will ensure the care planning process will deliver services within the person-centred, capacity building approach, and will assist consumers with the goal of independent living, by implementing various services that will meet the individual needs of the consumer and their lifestyle. 

To be successful in the role you will have:
• Tertiary qualification in a relevant Allied Health discipline, Social Sciences and/or relevant demonstrated experience in working with a broad range of consumers in a home or community setting, with an emphasis on capacity building, wellness, re-ablement and social inclusion
• Strong knowledge of the Commonwealth Home Support Programme, Home Care Programme and Home and Community Care Program for Younger People
• Experience in developing and facilitating the implementation of consumer care plans in line with consumer directed, wellness and reablement principles
• Excellent written and customer service skills and the ability to gain the cooperation of, and obtain information from people from diverse backgrounds and cultures. 
• Proficient in the use of Microsoft Office Suite, databases and knowledge of relevant client management systems such as Carelink+ and My Aged Care
